Requirements
Specific duties include, but are not limited to following:
·      New client processing, including greeting client, staff introductions, touring facility (inside and out), explaining rules and procedures, and answering questions.

·      Conduct searches at any given time, inspecting baggage and belongings; provide temporary clothing if the entire client’s clothing is being washed.
Â
·      Supervision and involvement in clients in day-to-day activities, grooming, meals, chores, administering medications.
Â
·      Continuously make rounds of the facility and grounds; no two mentors should be stationary in same area.
Â
·      Escorting clients to meetings, groups, sessions.
Â
·      Accompany clients to outside appointments
           Recording activities of clients
           Directly observing each client every 15 minutes during sleeping hours
Â
·      Administering restraints when directly supervised by a Counselor or Residential Supervisor, observing restrains and commitments.
Â
·      Demonstrates respect for clients and their rights; protects the clients’ rights to privacy and personal property; holds client’s personal information confidential, speaks positively towards clients; observes procedures for handling resident’s money.

·      Implements the Individual Service Plan specific to each client and providing appropriate and safe techniques in assisting with activities of daily living and independent living tasks (i.e. dressing, grooming, toileting, feeding, etc.).

·      Demonstrates knowledge and use of all learned information during the training videos during orientation (Confidentiality, Abuse and Neglect, Universal Precautions, Safety and Accident Prevention, and Infection Control).

·      Demonstrates safe transfer and ambulation techniques for each participant.
Â
·      Monitors medication and carries out other health related tasks after being trained by a licensed nurse.
Â
·      Act as a positive role model by interacting with the residents in ways that promote a good self-concept independence and age appropriateness.
Â
·      Shops or completes errands for clients as needed by the client, such as, doctor and therapy appointments, shopping, community inclusion activities, etc., and assists in securing or providing transportation in accordance with state laws.
Â
·      Assists with meal planning, preparation and eating and ensures adequate intake of nutrition and fluid intake.
Â
·      Immediately reports changes in clients’ body functioning to the licensed nurse. Recognizes emergency situations and the appropriate action to take.
Â
·      Performs household duties as assigned as it pertains to the client's plan. Maintains a clean and healthy environment.
Â
·      Assists the clients with developing and implementing community inclusion activities, as well as, facilitating social interaction and positive behavior.
Â
·      After each shift, detailed progress notes are completed on each client. Any important information will be verbally told to the oncoming shift and written in the logbook, i.e. medication changes, clients conditions, etc.
Â
·      Once an assignment is accepted, a 48 hour notice is required to cancel your assignment or disciplinary action may follow (except in emergency situations).

·      Participation in scheduled department and All Staff Meetings.
Â
·      Participate in outside agency training as directed by the Group Home Manager.
Â
·      Performs other tasks as assigned including all recommendations from counseling and therapeutic services, such as, positioning, range of motion, etc.
Â
Qualifications
Â
A Residential Mentor shall have the following:
Â
·      High School diploma or a General Education Development (GED) certificate.
Â
·      Must have good health in order to maintain an acceptable attendance record as established by the agency.
Â
·      Must be able to assist with lifting objects or residents (a minimum of 50 pounds for a one person lift and 150 pounds for a two person lift) utilizing approved techniques.
Â
·      All staff will be required to participate in prescribed annual training.
Â
·      The Residential Mentor must be able to work rotating shifts, and the ability to arrive in emergencies within 30 minutes.
Â
·      Bi-lingual is a plus.
Â
Persons in this position are required to maintain licenses in good standing, if applicable. All candidates must pass a Child Abuse Clearance and Pennsylvania Criminal Record search, plus an FBI criminal history clearance if resident outside Pennsylvania. All staff will be required to participate in prescribed annual training.
